Professional Registration Procedures

REGISTRATION AS AN INDIVIDUAL

Criteria:

• A full Health and Skincare qualification

• A minimum of two years relevant industry experience

Procedure:

1. Complete the Application form.  Please see notes on completing for any information you might require on completing the form.

2. Sign the agreement statement.

3. Fax or post the above with the following:

   • A copy of your identity document or passport
   • A copy of your qualification
   • A summarised CV of work experience
   • Proof of payment.  Initial registration amounts to R800 in total, which includes a once-off R500 evaluation fee and the annual R300 registration fee.  Bank details below.

REGISTRATION AS A CORPORATE/BUSINESS

1. Complete the Application form.  Please see notes on completing for any information you might require on completing the form.

2. Sign the agreement statement.

3. Fax or post the above with the following:

   • Proof of business registration
   • A rough floorplan
   • Lists of treatments offered (if applicable)
   • Proof of payment.  Initial registration amounts to R1000.00 in total, which includes a once-off R500.00 evaluation fee and the annual R500.00 registration fee.  Bank details below.

REGISTRATION AS A PROVIDER OF EDUCATION

1. Complete the Application form.  Should you require any assistance in completing the form, please do not hesitate to contact us.

2. Sign the agreement statement.

3. Fax or post the above with the following:

   • Proof of business registration
   • Proof of payment.  Initial registration amounts to R1000.00 in total, which includes a once-off R500.00 evaluation fee and the annual R500.00 registration fee.  Bank details below.
